How Ketamine is Helping Patients with Chronic Stress and Trauma

In functional medicine, we see trauma as more than just emotional distress; it has a tangible impact on both your physical and mental health. The Biology of Trauma

In recent years, there has been a growing interest in understanding the biological mechanisms underlying trauma and exploring innovative treatments. Dr. Stephen Porges introduced the Polyvagal Theory, a groundbreaking framework for understanding the autonomic nervous system’s role in responding to stress and trauma. According to this theory, the autonomic nervous system consists of three branches: the ventral vagal, sympathetic, and dorsal vagal systems.

  • Ventral Vagal System: Associated with social engagement and relaxation, this branch promotes feelings of safety and connection.
  • Sympathetic Nervous System: Known for its “fight or flight” response, the sympathetic system activates during stressful situations, preparing the body for action.
  • Dorsal Vagal System: Linked to immobilization and shutdown, this branch activates when an individual perceives a life-threatening situation, leading to a state of dissociation or freeze.

Understanding the interplay between these systems provides insight into how trauma can impact an individual’s physiological responses and overall well-being.

Trauma’s Impact on the Nervous System: 

When an individual experiences trauma, the autonomic nervous system may become dysregulated. Chronic exposure to stressful or traumatic events can lead to a heightened sympathetic response and an impaired ventral vagal system, hindering the ability to regulate emotions and navigate social interactions effectively. 

Additionally, trauma may result in an overactive dorsal vagal system, contributing to symptoms such as dissociation, anxiety, and depression. 

Understanding these neurobiological changes is crucial for developing effective interventions. Here’s several examples of patients* who’ve who’ve come to me for treatment:

  • The middle-aged patient with depression, who finally connects his dissociation and inability to focus, constipation, and recurrent sinus infections to the stress of his childhood-being raised by an alcoholic mother.
  • The 21 year old college student  who has diarrhea, difficulty tolerating foods, eczema and painful periods- who also lives in a state of fight or flight that she experiences as ADHD and anxiety.
  • The perimenopausal small business owner who is starting to have night sweats, weight gain, change in her bowels.
  • The sleep deprived mom of 3- shuffling kids from activity to activity, managing a household, cooking, who has difficulty finding time to exercise, meal prep, and meditate and who experiences chronic HIVES and cannot clear toxins like mold from her system.
  • The retired patient who has long haul covid after years of reflux symptoms, a short fuse and anger management issues, recurrent sinus infections, and a history of insomnia.  

Ketamine and Trauma Treatment

Ketamine, traditionally known as an anesthetic, has garnered attention for its potential therapeutic effects on trauma-related conditions. 

Recent research suggests that ketamine may influence the N-methyl-D-aspartate (NMDA) receptor,  a receptor in the brain leading to neuroplasticity and the formation of new neural connections.  

What this means is that the brain is literally rewiring itself, allowing faulty connections (synapses) to reconnect stronger and with more integrity. And what we experience physically is less anxiety and depression and the somatic manifestations of those states of being. 

Moreover, ketamine’s impact on glutamate transmission may play a role in dampening hyperactive neural circuits associated with trauma. 

Studies exploring the use of ketamine-assisted psychotherapy indicate promising results in alleviating symptoms of post-traumatic stress disorder (PTSD) and depression.  

I’m brought to my 20-something  patient who came to me, unable to work, in chronic pain, almost bed bound and with suicidal ideations.  After only 2-3 months of treatment, he is starting graduate school, his pain well integrated and understood, and walking with a new found confidence in himself.   Both having rewired the neural-circuits and integrated his experiences during his sessions into his daily life and conscious awareness.

What it’s Like to Experience a Ketamine Infusion

Launching the Ketamine Program  at Blum Center for Health with Dr. Danielle Greenman has been one of the most profound and exciting moments in my career as a Nurse Practitioner. When Dr. Greenman brought up the idea of bringing ketamine to Blum Center, I instantly became excited because I knew how life-changing ketamine can be.

We have seen some incredible results in our patients and overwhelmingly people have had a positive or productive experience. Ketamine is a wonderful option for so many reasons:

 

  1. Rapid Onset of Action: Ketamine is known for its rapid onset of action, often providing relief from symptoms such as anxiety, depression, rumination, low self-esteem etc. within hours or days, as opposed to weeks or months it can take for traditional antidepressants to become effective.
  2. Different mechanism of action: Ketamine acts on the glutamate system in the brain, which is different from the mechanisms of action of most other antidepressants (which primarily affect serotonin and norepinephrine). This unique action makes ketamine a great alternative or add-on for those who do not feel optimized on their current treatment.
  3. Neuroplasticity: Ketamine enhances neuroplasticity, which is the brain’s ability to adapt and reorganize itself. This can help people form new, more adaptive neural pathways and thought patterns.
  4. Greater mind body connection: The altered state of consciousness during the infusion leads to a heightened sense of introspection and self-awareness. This leads to a deeper connection to thoughts and emotions. Ketamine opens the door to the unconscious mind and allows people to explore what is lying beneath the surface. Emotions, thoughts, or memories that are difficult to reach in everyday life or an ordinary state we often don’t connect to are easier to grasp and process during a ketamine infusion. This can facilitate a deeper understanding of your emotional or past experiences and promote a more profound connection between your emotional and physical states. Ketamine can create a deeper self-awareness of thoughts, emotions, and bodily sensations that contributes to a stronger mind-body connection and a deeper sense of self.
  5. Reduction of suicidal thoughts: Ketamine has demonstrated the ability to rapidly reduce suicidal ideation. This could potentially be life-saving for those at high risk of self-harm or suicide.

 

It truly has been an honor to administer the ketamine infusions and walk alongside people on their journeys–a task that I do not take lightly. It is my mission to create as comfortable of an environment as possible to give people the safety they need to feel vulnerable and open to embrace anything that comes up in a non-ordinary state of consciousness similar to that of a deep meditation or hypnosis.

Before each infusion, I take a moment to set the intention of the experience with gentle reminders to start the journey off in a good headspace, where you are anchored to your body and have guidance for navigating the experience. Everyone is different and has different needs during the infusion and I honor whatever those needs may be. I take the time before someone starts their ketamine journey to make sure I understand how to best support them. 

There are things you can do to prepare to have the best experience during a ketamine infusion. 

How to prepare for a Ketamine Infusion at Blum Center for Health: 

  • Use the bathroom before you start the infusion
  • Wear comfortable clothes
  • Eat a light meal before
  • Bring an eye mask and blanket
  • Curate a playlist (we have a playlist for you if you need)
  • Communicate all needs 

Many people fear a “bad trip”. Dr. Greenman would say there is no such thing as a bad trip but rather our experience is shining a light on an area of the psyche that needs attention. What feels scary or dark to one may be exciting for another. That being said, when uncomfortable experiences present during a session either Dr. Greenman or myself will support you and help you find safety. Oftentimes what may have felt scary at first becomes enlightening. That being said, most people have joyous and happy experiences and do not experience anything uncomfortable at all. I am always amazed by the profound insights that people share at the end of the infusion and notice positive shifts in energy from when they first came in.

Every single person has dealt with something challenging in their lives and those are things we usually carry with us, whether we realize it or not. Ketamine leads to a greater understanding and connection to ourselves. We all have an organic inner healing intelligence and throughout the process, ketamine can help create a deeper sense of connection and understanding of this part of ourselves.

Is CoQ10 the Secret to a Healthy Heart?

CoQ10 Heart Health

When Heart health is talked about in a conventional setting, lowering cholesterol, quitting smoking, increasing exercise and reducing red meat intake is often recommended.  While we agree with those important steps for prevention or treatment of heart disease, the buck doesn’t stop there.  There’s many more building blocks that contribute to a healthy heart that we talk about from a Functional Medicine perspective.  

Mitochondrial health, your gut microbiome and hormone health can impact cardiovascular disease, too.  Inflammation will increase your risk of making plaque in all your arteries.  At Blum Center for Health, we treat the whole person and work with our patients to design a program to correct these underlying issues quickly and effectively. 

One treatment we often recommend is CoQ10 shots in our Infusion Center.  Because it is fat soluble and how well you absorb it is affected by the quality of your digestion, an injection is a great way to make sure you are getting what you need. 

CoQ10 is short for Coenzyme Q10, and is found in the highest concentrations in your heart and brain.  It powers up your mitochondria, which are the critical energy producing organs inside every cell of your body.  CoQ10 is also a potent antioxidant with many health benefits–heart health and beyond!

The whole approach to heart health and CoQ10

The prime candidate for CoQ10 injections are those who are taking a statin or red yeast rice for lowering their cholesterol, because both of these block CoQ10 production as a side effect of blocking cholesterol. Low CoQ10 levels are likely one factor responsible for the muscle aches and brain fog that are commonly reported in people taking statins especially.

If you have fatigue, taking a CoQ10 should be part of your program because it boosts mitochondrial production of energy. 

This is also why it’s so critical to take if you’ve had a heart attack or any damage to your heart or want to prevent damage to your heart, because it keeps those heart cells going strong.

What to expect when receiving a CoQ10 shot

The best way to receive CoQ10 is through our infusion center as an injection so you can absorb the entire dose. When given as an injection, it makes the CoQ10 100% bioavailable because it bypasses the GI tract and goes straight to the cell. 

The injection takes only a few seconds and is usually given in either the deltoid or gluteal muscle. There may be some pain when the injection is given and zero to mild soreness for a day in the muscle that the injection is given. 

We recommend that you get an injection once a week for four weeks and then to continue with maintenance injections every two to four weeks thereafter. 

CoQ10 is a very well tolerated injection with a good safety profile. Majority of people do not experience side effects but nausea, diarrhea, or heartburn have been reported.

It is important to note that CoQ10 should not replace any current medication regimen you are on, and can be helpful in addition to current medications or treatments. Do not stop any of your medications without discussing with your healthcare provider first. How You Can Benefit from Glutathione — the Ultimate Antioxidant

The benefits of antioxidants are undisputed – they slow the aging process and help prevent vision loss, cancer, heart disease, strokes, and other chronic diseases.  Most importantly, they help you feel vibrant and healthy!  Glutathione is known as the “master antioxidant” and is an extremely important molecule found in every human cell. It is considered the main protector against oxidative stress in the body. 

What is Oxidative Stress? 

Oxidative stress occurs when the ratio between antioxidants and free radicals are out of balance. Free radicals are molecules that are produced naturally in the body,  as a byproduct of normal metabolism or by exposure to environmental pollutants and stressors. Free radicals are molecules that are missing an electron, so they become activated and try to “borrow” electrons from other molecules in healthy cells.  This borrowing action damages the cells themselves and is often the earliest trigger that starts inflammation. 

Think of oxidative stress as a situation when your antioxidants aren’t keeping up, and then these free radicals can create little sparks or embers of inflammation throughout the body. When you are sick, stressed or come in contact with pollutants, your body produces even more free radicals – and in turn more oxidative stress – that can damage your DNA, cause inflammation, contribute to disease and accelerate aging.

How Glutathione Combats Oxidative Stress

Your body comes equipped with antioxidants, especially glutathione, to neutralize these free radicals. Antioxidants work to balance the load of free radicals and create a harmonious, healthy environment for you to thrive. The two ways nature gave us to prevent oxidative stress is through food and our natural antioxidant systems in the body of which glutathione is the champion. That’s why eating a diet with a variety of colorful  fruits and vegetables is so important! 

Additionally, the body naturally creates glutathione to balance and repair damaged cells. Glutathione is critical for the function of the liver to detoxify and works to put out the “fires” that free radicals have sparked. Glutathione is a tripeptide of three amino acids (glycine, cysteine, and glutamic acid). This antioxidant helps your body fight disease and injury by binding to free radicals and facilitating their removal from your body.

Under certain conditions you may have a higher demand for antioxidants. There are several factors that can significantly use up and decrease your antioxidant levels, thus increasing your need, including: 

  • high toxic burden 
  • poor diet

  • Infections, including gut dysbiosis

  • Pollution
  • Smoking
  • Alcohol
  • certain medications
  • excess body fat
  • age 

The use of Functional Medicine testing can measure the amount of oxidative stress you may be carrying. A session with our Health Coach or one of our Functional Medicine Physicians can help determine your best course of action.  To replenish glutathione, we typically recommend intravenous therapy as it completely bypasses the digestive system and delivers the antioxidant directly to your cells.  When taken as an oral supplement, glutathione is not typically absorbed well as it is destroyed by digestion in the stomach.

Is NAD+ The Answer to Your Fatigue?

How Infusions Can Help you Recover from Lyme Disease

I don’t know about you, but I LOVE living in the Westchester area, especially in the summer. Crisp evenings, proximity to beaches, and embracing the outdoors brings me so much joy. However, one thing I don’t love about Westchester in the spring, summer and fall are the Lyme disease-carrying-ticks that are everywhere.  The official name of the microbe that causes Lyme is Borrelia Bergdorfi.  And these annoying and harmful pests carry other infections, too, like Bartonella, Babesia, Ehrlichia and more.  Collectively we call these Tick Borne Illnesses or TBI’s.  New York, New Jersey and Connecticut are three of the ten states where the majority of Lyme cases are reported in the United States!  The problem is that we all love the outdoors and your favorite summer activities could be putting you at risk.

Most of you probably already know this and check yourself for ticks after every evening beach walk or nature walk.  If you don’t, then this is definitely your first line of defense.  If you find a tick on your body and it has been there for at least 4 hours, and has hooked itself in your skin, remove gently with a tweezer, and call your doctor.  You can save the tick and have it tested for Lyme.  At Blum Center we believe in treating aggressively, and recommend antibiotics (or sometimes we use herbal remedies) whether or not you develop a rash.  If that tick has latched onto you, whether or not is engorged or swollen, then treating right away is your best defense against developing acute or chronic Lyme disease.  

ACUTE AND CHRONIC LYME

Acute Lyme or Tick Borne Illness (TBI) usually happens in about 2-3 weeks after the infection, but can happen sooner or later than that too.  You might have a fever, chills, headaches, muscle or joint pain.  It can feel like a flu.  It can last 5-7 days and then go away.  The tricky thing is that some people who get infected by a tick never have an acute illness!  And many people with an acute TBI don’t ever remember having a tick bite!

That’s why many people (and doctors) don’t even think about Lyme when they develop chronic symptoms like fatigue, arthritis, muscle pain or brain fog that drag on for months and months.  There can also be other symptoms like intermittent fevers and heachaches and neurological symptoms like strange sensations and/or numbness or tingling anywhere in the body.  The good news is that WE do think about Lyme in every patient with these symptoms at Blum Center and do the testing and pick up hidden Lyme or Tick infections all the time.  

The name Chronic Lyme has been given to people who have these symptoms for months and sometimes years.  Some people have Chronic Lyme and don’t know it.  And even when they start treatment with antibiotics, they continue suffering with Chronic Lyme because once it’s been in your body for a long time, it’s very hard to get rid of it, even with antibiotics.

FUNCTIONAL MEDICINE FOR LYME TREATMENT 

Treating your Lyme disease requires a very personalized approach, and our complete treatment program for Lyme disease is beyond the scope of this blog.  We use at different times, and for different people, antibiotics and herbal programs to kill the Borrelia Burgdorferi (or other TBI) microbe.  But treatment is a 1-2 punch:  the medication or herbs lower the total amount of infection in your system, and then the immune system has to clear out the rest.  You can see why a highly functioning and strong immune system is critical to treating Lyme Disease.

In addition to a strong immune system, once we start treating Lyme, there are a lot of toxins that are released and can cause you to feel very sick.  And these toxins and the Lyme itself can wreak havoc with your mitochondria, the little furnaces in your cells that make energy.  Therefore from a functional medicine perspective, whether you are currently being treated by us or other doctors, or are still recovering from Lyme Disease, you should add to your program support for the immune system, the detox system and the mitochondria.  And the most effective way to do this is with Infusions because high concentrations of the needed nutrients can go right into your blood and to your cells where they are needed.

How Infusions Can Protect You from Covid and Post Covid Infections

I’m sure we all know a handful of people who got Covid from this most recent surge. Whether you just had it or have “dodged the bullet” for the last two years,  it’s getting harder and harder to avoid it now.  The good news?  If you are vaccinated and not immunocompromised, you are unlikely to experience a severe illness that will put you in the hospital. ANTI-AGING NEWS! IS NAD+ THE NEWEST FOUNTAIN OF YOUTH?

We are excited to introduce  NAD+ to our infusion menu.  It is the latest darling of the anti-aging movement!

NAD+ and ENERGY (MITOCHONDRIA)
